Initial Assessment
First visits are typically 30-60 minutes in length dependent upon your visit type
A complete thorough assessment for the reason of your visit will take place. Once your assessment is complete, we would then discuss your diagnosis and treatment recommendations. Recommendations may include a combination of medication, therapy, and additional support. If you choose to take a medication your medication will be electronically provided to your pharmacy of choice along with follow-up recommendations.
Follow-Up Visit
Follow-ups are typically 10-30 minutes in length dependent upon your visit type
Follow-ups are regular check-ins with your Provider to see how things are going. We’ll review your progress, check in on your symptoms, make sure you aren’t having any side effects, and work together in developing a course of action to get you where you want to be with your health. Follow-up visits can vary in frequency from every two weeks to up to six months based on your progress in your treatment.
In-Person Visit
Prior to arriving you will have the capability to check in via phone, complete insurance verification if applicable and complete consent forms with our secure online site Phreesia. When you arrive, you will be greeted by your scheduled provider to begin your appointment without a wait.
Telehealth
Have your visit with us from the comfort of your home! Telehealth visits are completed over a HIPPA compliant
audio-video platform. A good internet connection, a device with quality audio and video capabilities, and a confidential location are required. You will receive instructions on how to access your visit prior to your appointment.

We have simplified our booking process and initial appointments can be scheduled either with our online booking system or by phone with one of our staff members.
1) Online Booking
Our online booking with Kareo is easy to navigate. A payment or copayment may be required to schedule an appointment.
2) Booking With Staff
Appointments requested by phone are completed during regular business office hours or you may leave a voice message for a return call to schedule.
3) Same Day Visits
Same day appointments are available as the schedule permits.
Plan to arrive 15 minutes before your scheduled appointment. To help prevent delay of your scheduled visit it's recommended for you to complete and sign all forms at least 24hours prior to your visit. If forms are not completed on time your scheduled appointment may be subject to cancellation or rescheduling and fees might apply.
If you are more than 10 minutes late to your appointment:
* The visit will be considered a late appointment, and you will be asked to reschedule, and fees will apply.
We understand there are times that emergencies and circumstances out of one's control may happen, considerations will be made on a case-by-case basis. Any and all in-person or virtual sessions with a minor, a parent or legal guardian MUST be present to consent to treatment, provide history, and discuss/agree on a treatment plan.
Book all your future follow-up appointments online in a timely manner to avoid interruption in your care. Same day appointments are available on a first come first serve basis on our online scheduling system Phreesia. An invoice will be provided upon receipt of payment, we will then proceed with scheduling an appointment.
We continue to offer telehealth appointments out of the convenience of your home or office. For safety and confidentiality reasons we DO NOT conduct telehealth visits while patients are in public settings or while they are operating a motor vehicle(driving). A HIPPA compliant telehealth link will be sent to you before your appointment time. Please ensure you have a good Wi-Fi connection, so the session does not get dropped. Dropped sessions may be subject to appointment cancellation fees. Our staff is always available to assist you with this process or answer any questions you might have.
Your appointment time is reserved exclusively for you. A 24-hour notice is required to cancel or reschedule any appointment to avoid any fees. This allows us to offer that appointment time to another patient in need.
All Initial visits cancelled or rescheduled less 24 hours will be charged a nonrefundable fee of $100. All established patient appointments cancelled or rescheduled less than 24 hours will be charged a nonrefundable fee of $50. These fees apply to No Show or late appointments (over 10mins late).
Any appointment rescheduled or cancelled up to 3 times, the patient will be set to WALK-IN status only and no scheduled appointments will be completed on their behalf.
We only refill medications during appointments. Unforeseen circumstances happen and sometimes patients find themselves running out of medications. We strongly encourage patients to make follow-up appointments or show up for scheduled appointments to avoid interruption in their care. We do charge a fee to provide bridge supply of medications or a 30days supply script. This is time away from other duties to review your chart and make a proper clinical decision. Allow up to 24hours for such requests to be completed.
We do assist our patients with completing or creating documentation/paperwork on their behalf. We do charge an administrative fee each time any of such tasks is completed. Our turnaround time is 24hours to 5days depending on the complexity of the case. These are the different reasons for which we will perform these tasks on your behalf:
